Heart-treated Autogenous Bone Graft with Intramedullary Vascularized Fibular Graft in the Treatment of Osteosarcoma

Abstract
The advancement of diagnostic techniques has allowed more accurate preoperative assessment of the extent of a tumor. With the development of chemotherapy and anatomical staging, the limb-salvage operation has recently become more popular. There are several methods for reconstructing the large bone and soft tissue defect after radical resection of malignant tumors, such as tumor prosthesis, heat-treated autogenous bone graft, allograft and rotationplasties etc. Many studies have reported that low-heat is far better than autoclaving over 100¡É for the preservation of the bone morphogenic protein and mechanical strength of heat-treated bone. Based on these assumptions, the authors used heat-treated[65¡É, 30 minutes] autogenous bone graft supplemented by intramedullary vascularized fibular graft for additional mechanical strength and rapid union. The clinical and radiological results of the two patients who were treated the osteosarcoma in the lower extremities from January 1996 to May 1997 were studied. The ages were 15 and 17 years old in each case and the follow-up period was 12 and 14 months in each case. The duration of immobilization was 8 and 6 weeks, respectively. The time to union between the graft and host bone were averaged 5.5 months. There was no evidence of local recurrence or distant metastasis during the follow-up periods. In this study, we concluded that heat-treated autogenous bone graft with intramedullary vascularized fibular graft provided more stability, more rapid union and may be one of the best curative reconstruction methods in the limb salvage surgery of osteosarcoma.
